With the card information you can add the card to your dave and busters account through the app and transfer over any tickets to a card that is already added on the account. If you dont have another card, go to any dave and busters location with the card already on your account and go to the front desk and say you would like a new powercard to replace the one on your account and to transfer the entire balance on to the new card, they should be able to help with the rest.
